This checklist is a planning aid. Use the current Board application and instructions as the controlling source.
Personal application planning checklist
- Confirm that the personal license category matches the work you plan to perform.
- Download the current official application and forms.
- Complete and print the ALEA release form. The Board forms page says it is required with applications other than the Certified Trainer Application.
- Prepare two fingerprint cards. The Board forms page says two sets are required with applications except Certified Trainers.
- Complete any training that applies to your path and keep the completion documentation.
- Review the current application for fees, required supporting documents, signatures, and submission instructions.
- Keep a copy of the complete submission for your records.
Before you submit
Check every item against the current official form. Do not send an application, fingerprint cards, fees, or sensitive personal information to ASRBTraining.com or any training provider.
