What the Board forms page currently says
The Board states that two sets of fingerprint cards are required with all applications except Certified Trainer applications. It also states that applications other than the Certified Trainer Application must include a completed and printed ALEA release form.

- Read the current fingerprint-card and ALEA-release instructions in full.
- Use the card type, identifying information, and fingerprint service required by the official instructions.
- Review the application for the correct number of cards, supporting documents, and mailing or submission method.
- Protect all fingerprint and identity documents; submit them only through the official method.
Do not upload fingerprint cards, Social Security numbers, identification documents, or background information to ASRBTraining.com.
